Semax (Met-Glu-His-Phe-Pro-Gly-Pro) is a synthetic heptapeptide analog of the adrenocorticotropic hormone fragment ACTH(4-10), composed of the original ACTH(4-7) fragment with a C-terminal Pro-Gly-Pro tripeptide extension. Research has established that Semax rapidly increases brain-derived neurotrophic factor (BDNF) protein levels (1.4-fold) and TrkB receptor tyrosine phosphorylation (1.6-fold), alongside a 3-fold increase of exon III BDNF and 2-fold increase of TrkB mRNA levels in the hippocampus — mechanisms considered key to its neuroprotective and cognitive-enhancing activity. The peptide influences multiple neurotransmitter systems, including dopamine, serotonin, and norepinephrine, contributing to its diverse neurological profile. Studies have demonstrated Semax’s potential to improve selective attention, modulate brain development, stimulate anxiolytic and antidepressant effects through activation of the brain serotonergic system, and mitigate copper-induced cytotoxicity in neuronal models. In ischemic cortex models, Semax treatment promoted earlier upregulation of BDNF, TrkC, and TrkA transcription, and prevented the decrease in Nt-3 and NGF expression, suggesting pronounced neuroprotective activity under conditions of cerebrovascular stress.
