SS-31 (Elamipretide, also known as Bendavia and MTP-131) is a novel synthetic mitochondrial-targeted tetrapeptide with the sequence D-Arg-Dmt-Lys-Phe-NH2, featuring an alternating pattern of basic and aromatic amino acids. This structure allows SS-31 to readily cross the blood-brain barrier and cell membranes, accumulating on the inner mitochondrial membrane at concentrations exceeding 1000-fold. SS-31 selectively interacts with cardiolipin, a unique phospholipid located in the inner mitochondrial membrane critical to mitochondrial respiration, where it inhibits peroxidase activity of cytochrome c. This mechanism reduces reactive oxygen species (ROS) production from the respiratory chain while simultaneously enhancing electron transfer and improving bioenergetic output — increasing ATP synthesis and optimizing mitochondrial electron transport. Preclinical studies demonstrated that SS-31 protected cognitive function and mitochondrial complex I activity, maintained mitochondrial membrane potential, facilitated BDNF signaling, and restored synaptic proteins in neurological models. Research supports SS-31’s capacity to improve ADP sensitivity in aging muscle mitochondria through the adenine nucleotide translocator (ANT), leading to increased ATP production and improved physiological function. SS-31 has advanced into multiple clinical trials addressing mitochondrial myopathy, heart failure, renal perfusion, and age-related skeletal muscle decline.
